My greatest reservation about your proposed model for mental health is that it appears to dissociate mental and physical health.  This separation appears to give privilege to specific mental states.  I mean those mental states concerning the brain, mind, and soul as opposed to the body or physical experiences.  However, I do not think that mental health can be so restricted.  Consider the topic of pain management.  The mental states that accompany the experience of presence and absence of pain.  The anguished anticipation for relief by a narcotic and strong sense of relief following.  It seems like a roller coaster ride of moods.
