Stem Cells Cure Blindness

The Controversy

Earlier this November, scientists from the University College London Institutes of Ophthalmology and Child Health and Moorfields Eye Hospital were able to restore vision to blind lab mice. This scientific breakthrough signifies that millions of people with optical conditions such as macular degeneration (loss of sight experienced by the elderly), diabetic retinopathy, and a variety of other forms of blindness could be able to regain sight through a remarkably simple procedure. However, the fact that the procedure requires stem cells from foetuses—currently viewed as a highly controversial method by many politicians—has prevented this procedure from becoming more publicized in the U.S. (1).

The Breakthrough

Researchers have identified certain cells on the margin of adult retinas that are similar to stem cells. Additionally, retinal cell replacement may be the most effective method of "cell transplant therapy because photoreceptor loss initially leaves the rest of the wiring to the brain intact” (1). In other words, major surgical reconstruction is not necessary. Any surgical procedure would only involve the superficial layer of the retina and not the particularly sensitive optic nerve wiring at the back of the eye. However, in order to attain human retinal cells at the necessary stage of development, stem cells would need to be extracted from a foetus during the second trimester of pregnancy (1). Because stem cells are able to proliferate and develop into many other types of cells within the human body, they can be extracted from any part of the foetus. However, the timing is imperative if the procedure is to work.

Three Blind Mice… See How the Procedure Works...

1. Early stage retinal stem cells were extracted from a 3 to 5 day old newborn mouse (1).

2. The retinal cells were transplanted onto the retinal surface of a blind mouse whose condition was genetically programmed to resemble the gradual loss of sight characteristic to the human disease retinitis pigmentosa or age-related macular degeneration (1).

3. The cells embed themselves and connected with other cells on the retina of blind mouse. Within 30 minutes the photoreceptors from the retinal stem cells implanted themselves and fused electrical connections with the animals' existing retinal nerve cells (3). As a result, the formerly blind mice's pupils began to respond to light and there was activity in the optic nerve (indicating that the eye was transmitting signals to the brain) (1).

Anatomy & Physiology of the Eye: Photoreceptors

The retina (around 0.5 mm thick) lines the back of the eye. It is lined with a network vascular blood vessels and neurons that gradually channel towards the optic nerve which contains the ganglion cell axons that connect the ganglion cells to the brain. The ganglion cells—the neurons of the retina that transmit images to the brain—are located in the innermost region of the retina and extend toward the lens, or anterior portion of the eye (2). The photoreceptors—the rod and cone shaped cells—are situated toward the outermost portion of the retina and are closer to the back of the eye (5). As a result, light must penetrate the nerve cells within the retina before reaching and activating the rods and cones. Once reached, the rods and cones absorb photons through their visual pigments and translate the photons into a biochemical message and then into an electrical message that stimulates all of the succeeding neurons of the retina. Consequently, “the retinal message concerning the photic input and some preliminary organization of the visual image into several forms of sensation are transmitted to the brain [by] the spiking discharge pattern of the ganglion cells” (2). From then on the brain is responsible for identifying, processing and interpreting the visual image (2).

Candidates for retinal cell replacement surgery must have some retinal cone and rod photoreceptors intact (1) . The retinal cell replacement surgery primarily serves to repair the nerve synapses in the retina, the macula lutea, and the fovea. The surgery cannot generate new photoreceptors.

The surgery mainly repairs the macula and the fovea. The macula functions as a short wavelength filter while the fovea, characterized by a dark circular area towards the back of the eye, is considered to be the most vital portion of the retina. Like the lens, it functions as “a protective mechanisms for avoiding bright light and especially ultraviolet irradiation damage” (2). The fovea is entirely composed of a mosaic cone photoreceptors that are arranged in a hexagonal structure. Outside of the foveal pit, the density of cone photoreceptors becomes increasingly more balanced with that of rod photoreceptors. There is a peak in the density of rod photoreceptors at about 4.5mm (or 18 degrees) from the foveal pit where the rod photoreceptors arrange themselves in a ring around the fovea (5). (Naturally, the optic nerve (the blindspot) is entirely free of photoreceptors) (5). If the macula or fovea cones are damaged (as happens gradually over many years), instant blindness results (2). However, macular degeneration could be easily remedied since stem cells take only about half an hour to develop into photoreceptors.

Repairing the Cornea

In August of 2003, Mike May, a Californian man who had been left blind for 40 years as the result of an accident that happened when he was three years old had his vision restored. Though the vision in his left eye was permanently lost, he could still sense light with his right eye. Researchers implanted corneal and limbal stem cells into his right eye. Five months after the surgery, May was able to sense movements and recognize simple shapes. After two years, he was able to see forms, color, and motion nearly accurately. His 3D perception and face and object recognition remained impaired, though his ability to sense motion was the best restored visual faculty (4).

Like photoreceptors, the cornea is responsible for channelling light through the eye's surface. The corneal surface refracts to provide 2/3 of the eye's focusing power. he corneal surface is entirely transparent and not lined with blood vessels, so the uniformity of cells may contribute to its ability to regenerate more rapidly than other cells in the human body. On the other hand, it is extremely sensitive. There are more nerve endings on the cornea than anywhere else on the human body (6). The cells that compose the layers of the cornea are found to regenerate at a rapid pace, though less rapid than photoreceptor cells. Again, a simple surgical procedure, most of which is processed by human mechanisms, could restore sight to millions of people if only the procedure were to be legalized.

Stroke of the eye and optic Nerve

My brother in law is 55 and had a stroke of the optic nerve just before Thanksgiving in his right eye. It left him totally blind in that eye. It was caused from a peice of plaque breaking loose in the carodic artery and lodging behind the eye. Unfortunately, that was his only good eye he could see out of. His left eye is an amblyopic eye from childhood and he can only see shadows and vision is quite blurry and makes everyday life very difficult.

Is there anything out there you know about that could help the amblyopic eye see better and or any help with the eye that had the stroke and remove the plaque and get blood flow and oxyegen back. We know Europe is further ahead in the science field than the US and we are looking for any answers that may give us hope for some better sight in either eye.

Optic Nerve Damage

My daughter is 15 months old. She had a stroke in utero. As a result, she has her optic nerve damaged and it is pale. Because of her age, we are not sure how well she can see, but we do know that she has some bonocular vision. She grabs things and follows objects slowly. We live in the US and were told that as of today there was no treatment available to repair the optic nerve. I would greatly appreciate it, if you provide us with information as to whether your treatment may benefit her, cost etc.

I have 19 yrs old daughter suffering from retinis pigmentosa for the last 9 yrs. She can see in daylight but cant see in dim light n in the night. I am interested in cell transplant therapy. I am a twenty yrs old boy

I am a twenty yrs old boy from norway. My right eye has been "blacked out" my whole life due to some sort of ONH (Optic Nerve Hypoplasia). I want to operate because I want to be a policeman or serve in the military. anyone who knows if this can be done without major expenses? I have heard something about a clinic in china, but that´s like 70 000 USD or something...

I´ve seen TV reports of babies hwo have went through this type of operation and gotten their vision fully restored- however, is it possible that it´s slightly more complicated on young adults? Can this be done at all?

Damaged optic nerve due to brain tumor removal

8 and 1/2 years ago, a benign manengioma brain tumor was found in my left frontal lobe. Less than one month after the discovery of the tumor, a surgery took place to remove the calsified tumor. After the surgery, there were major unexpected complications due to the removal of the hard tumor. When it was cut out, a lot of my memory was wyped clean, and had to be rediscovered somewhere else in my memories. The other major problem was the damage of my optic nerve. Since the surgery, of Apr 28, 2001, I am blind to the right of center in both eyes. My memories have improved greatly since the brain surgery, yet my vision has not. Note: I do notice bright lights to my right. With this knowledge of my situation, is there a possibility that someday soon, with the help of the stem cell actions, that my right side peripherals of both eyes could be brought back to life? If there is any knowledge of this, please share the info with me so that I can start to regain my vision that I used to have. Any info will be greatly appreciated.

Hi, my name is Elise and my husband is John. I am 55 and my husand is 63 years of age. My husband and I have four children, the youngest is a 14 year old boy. My husand john is totally blind in right eye and cand barely see out of his left do to diabeties, and something to do with the optic nerve I believe. Its been years and its been very hard for my youngest son Jesse. He sees all the other friends doing things with thier father and his father can't do to triple bypass, having a bypass in his leg making it differcult for him to walk fast without tripping. Optic Nueritis

My daughter first came down with acute ON when she was 14, Due to the insurance company fighting us on the proper course of treatment(they said none was needed) she lost the vision in her right eye. She had a relapse of it 5 years later in her left eye and we stopped it using soluemedrol. She has been fine now for 10 years until she was put on a birth control called Ocella. This stuff apparently can cause ON to reappear in those that have had it before. We have it stopped for now but it is acting like it is coming back. She has optic nerve damage in her right eye...dead. We are now concerned she may have trouble with the right eye if this keeps coming back on her. Can this treatment regenerate the dead optic nerve??? Can it help.

BBS type 1

My son has a genetic syndrome called BBS type 1. I am being told that he will lose his sight into his teenage yrs. Please if anyone knows any research or treatments anywhere being done today let me know. He is only 3 yrs old now, so doc are hopeful that something will work one day to cure the blindness, but have told me to prepare for the onset of blindness. I noticed tonight that he can't see well in the dark and that is a sign that his sight is diminishing. I am very scared and feel extremely hopeless.

BLINDNESS IN A FRIEND AND STEM CELL TREATMENT?

I came across this site in Googling. I have a friend who has been legally blind since birth who recently lost his remaining vision with exception of light-dark sensation to glaucoma and now has "pale disks" and optic nerve atrophy.

I was wondering if stem cell treatment in China could help regain his sight.

His earlier vision problems were due to retinal dysplasia caused possibly by retrolental fibroplasia (present from birth), but he was stil able to see and watch TV; now he cannot do those things.

My friend underwent severe pressure and went in to coma. After recovering from the illness we find that both her eye vision is damaged while doctor says that due to excess pressure the retina nerves have got damaged, Hence only stem cells can help her out in getting vision.

I have got an arrow shot at my right eye when I was 3 yrs old, at dat tym my eye ws operated for cataract.Gradually, the eye sight of the right eye became worse and now its no more vision in my right eye.Few years back I got to knw dat it has becum the case of Glaucoma.The eye pressure of my eye remains very high (around 40 or above)inspite of taking the eye drop regularly , which causes me severe pain also,sometimes.Kindly suggest me, if there is any treatment (especially to maintain the eye pressure of my eye)so that I can live a painless life ahead.

Please can anyone offer advice?

My father was beaten trying to save a lady from being attacked, he is now blind in his left eye, they jumped on his face causing the eyeball to explode. it has been 5 months now and he gets the odd light sensation but can not see anything, he is extremly short sighted in his right eye and can see almost nothing. This is destroying my family and there seems little anyone can do. Does anyone please please have any suggestions that could help us.

high myopia and retinal scarring

my grandson is 5 years old he has a negative -12in reight eye and -13 diopters in the other eye which reads as 20/200 one eye 20/300 other, 20/160 with glasses and both eyes working to gether. he also has scarring in his retinas the drs say they are not sure how much he really sees. they are now saying he sees a lattice affect. we go to iowa university hospitols and clinics he has no known sydrome he gets around quite well even rides a bike he is truly amazing to the drs. has anyone heard of such or been treated with stem cells to correct retina damage?

Optical Atrotry

Hi

I am 31 year old. I am loosing my eye sight and the Doctor told me that my optic nerf swells and peels. He called my problem "Optical atrophy" and he told me that there is no cure for damage nerf. iam dorji from Bhutan,since 2008 my child has been suffering from optic nerves,i mean retina and i have attaching my medical repots also.here in bhutan the eye specialist said there is no cure. is there any cure through stem cells,let me know sir .
