The DSM and its revision

"Is this new edition, the DSM-V, really needed and how valid and reliable is it and past manuals?"

My sense is that there are two at least somewhat distinct questions here.  One is the pros and cons of a DSM type manual, and the other is whether proposed revisions of the existing manual will make it in some ways better than it has in the past.  I have to admit I'm a little skeptical on both counts, given the ways usage of the DSM has evolved over the years since its first appearance.
