Who decides what constitutes

Who decides what constitutes an “underlying problem?” If a life-long smoker develops health problems or someone morbidly obese does, should these people not be offered care, if an “underlying problem” cannot be determined? I say they should be offered care. Why is mental health any less "valid?"
